§ 29-113. Registration information.

The information required for registration includes the following:

(1)

Identity and legal status of ROW user and names of all operators of any facilities on the right of way.

(2)

Name, address, telephone number, fax number and email address of officer, agent or employee responsible for the accuracy of the registration information.

(3)

Name, address, telephone number, fax number and email address of the local representative of the ROW user who shall be available at all times to act on behalf of the ROW user in the event of an emergency.

(4)

If applicable, certification number issued by public utilities commission.

(5)

General description of services to be provided.

(6)

Insurance and bonding information.

(7)

Employee safety certification information.
