§ 33-053. Notification of change of address or ownership.

A holder commits a violation if the holder:
(a)

Fails to notify the director within five (5) business days of a change in the address or telephone number of the holder; or

(b)

Changes the form of the business or officers of the corporation of the ground transportation service from that originally submitted, without a request to amend the operating permit that is approved by the director.
